We understand arriving at a chiropractic office for the first time can be an anxious experience, especially if you've never experienced chiropractic care. Maddas Chiropractic & Rehab desires to create your first visit with ease. We make it easy for patients to complete all patient forms online.
We offer you the opportunity to complete your forms in the comfort of your own home. Just click on the heading "New Patient Center" and from the dropdown select "Forms." Choose the form(s) by downloading, complete the form(s) requested information, save them, and attach the forms to our secure office email address in the "Contact" section or you may print the completed forms and bring them with you, along with your insurance information and drivers' license to your scheduled appointment.
Maddas Chiropractic & Rehab Center wants to offer you ease, convenience and comfort upon your arrival, relieving any stress of your chiropractic visit. We also understand, arriving on your first chiropractic visit your expectations of learning new faces, getting your way around the office and experience with Dr. Maddas could be overwhelming.
That's why at Maddas Chiropractic & Rehab Center our aim is to remove any unnecessary stress or tension to make you feel comfortable during your visit with our chiropractor and staff. Our staff and doctors are focused and attentive when it comes to our patients care!
Upon entering our office, our receptionist will welcome you as a member of our family. We will request the completed patient forms located in the "New Patient Center" section of this website. This paperwork provides us with a detailed health history and information on your condition. If you didn't use our online forms, no problem! You can complete these forms in the office before your treatment; however, we ask if you will arrive 15 minutes prior to scheduled your appointment time.
Consultation
You will then consult with Dr. Aaron Maddas to discuss the reason for your visit, health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Maddas to learn more about you, your condition, treatment plans and future expectations of how chiropractic care and health awareness can meet your goals.
EXAMINATION
After your consultation, Dr. Maddas will perform the chiropractic examination checking your reflexes, flexibility, standard neurological, orthopedic, postural, and physical examination.
TREATMENT
After, the examination Dr. Maddas will perform your first treatment. This may include spinal adjustments, physical therapy, and/or soft tissue massage. This treatment is interactive, so feel free to express your concerns at any time.
IMAGES
Depending on your condition, we may request X-RAYS, MRI, or CT images to determine the most effective treatment plan for you; this can also alert us of any serious conditions.
REPORT FINDINGS
When your image information has been received, Dr. Maddas will review this information with you and give you a detailed report of all findings and answer any questions you may have. Dr. Maddas will discuss recommendations and inform you if your condition will require additional care with other providers. Dr. Maddas will provide the best treatment plan for your needs.
WELLNESS PLAN
At the completion of your visit with Dr. Aaron Maddas, he may suggest a wellness plan to incorporate with your treatment.
Depending upon your condition and physical state this may include ice or heat application instructions, certain activities or positions to avoid, home exercises and/or stretches. If you desire, Dr. Maddas will work with you to create healthy habits and routines for your lifestyle. He may suggest nutritional supplements, as well; however, each person is unique and will require a customized wellness plan.
Our wellness plans are for each individual to achieve overall health, good spinal alignment, create a healthy diet, exercise routine, and maintain positive mental health.
